Starting ABA therapy is an important step for your child and family. To help us create the best plan of care, we require a few intake forms to be completed before services begin.

Complete the Forms
Provide your child’s details, medical history, insurance info, and any prior evaluations. Don’t worry if you’re unsure – we’ll review everything with you.
Submit Securely
Send your completed forms via our HIPAA-compliant SendSafe link. If you prefer, forms sent from personal email accounts are received securely once they reach us.
